Yea this was a lipemic sample, classic for hypertriglyceridemia, not hypercholesterolemia. Most likely familial when you see numbers are high as his (2300 mg/dL!) but one should consider secondary causes too: EtOH, hypothyroidism, medications (for women, BCP esp), uncontrolled diabetes. This patient also has a history of recurrent pancreatitis, undoubtedly from the very high Tg. Despite medications, there is a good chance this pt will still need plasmapheresis to get things to a satisfactory level.
